Prescribing Decisions

Addressing treatment nonadherence in children with ADHD

Long-acting stimulants are much preferred for ease of use and better patient compliance. I make sure before starting meds that all adult caretakers understand benefits and are on board with giving these meds. We discuss safety issues and side effects in detail; in particular, that these meds are not habit-forming when used correctly. It's often helpful if they can keep an extra bottle at school for school to administer in the morning on weekdays (pharmacy splits rx into multiple bottles.)

Diagnosing cow’s milk allergy in infants

For our infants, we start by evaluation with thorough history and physical. When an infant has a hx of poor tolerance of feeds, increased fussiness and gassiness and recurrent rashes as well as a history suspicious for blood-streaked stools, I'll start with testing for fecal occult blood. If this is positive, we will start with a trial of eliminating milk and soy from mother's diet if a breastfed infant or with eHF formula if infant is formula-fed. Consider allergy testing or referral to GI if symptoms don't resolve after several week trial of maternal elimination or eHF formula diet.
