I think a big barrier is in physician education on importance not just of starting the correct meds but titrating up. I see many patients who are started on a low dose, the prescribing doctor checks the box, and they are never titrated. As others have mentioned, this requires a lot of time and attention, so EMR-based prompts and alerts would be a helpful adjunct. I could imagine a pop up alerting the physician to a suboptimal dose and requesting a reason for low dose. If there is a good reason entered (like BP or kidney function won't tolerate), the pop up would stop being presented.

Addressing treatment nonadherence in children with ADHD

I'm a bit surprised that medication adherence is found to be low among adolescents. In my experience, they are actually compliant in terms of taking their ADHD medication regularly. Counselling starts before we even write the prescription. Discuss any potential side effects so patient and caregivers are aware. Also, invite them to voice all their concerns and ask any questions. This will lessen any barriers that they may have regarding the medication.

I try to address A1c and weight reduction as soon as possible. There are many factors to evaluate before choosing therapy. Other co-morbidities, current medication, previous medicines, allergies, patient preferences, insurance coverage, cost out of pocket, dosing convenience, goals and objectives.

I have practiced Family Medicine for over 35 years. Hyperlipidemia is such an important health issue that it does take a multidisciplinary team. I think it’s helpful for a patient to hear from multiple providers how important it is to get the LDL to goal. I welcome the input from the specialist in this realm, but always keep a very close eye on this medical issue.

Redefining Patient Identification for Obesity Management

Obesity as we practice now all pivots around BMI and that where the Stigma of it comes in the picture but need to look beyond it and more profoundly in to the overall health of the pt. Whereas you need to address the weight of the patient for all the right reasons to manage it realistically and practically, attention needs to be paid to waist to hip ratio to stratify the health risks and pts should be encouraged to reduce it as it occurs in particular when they are exercising and the weight increases but the ratio decreases. In addition to that attention should be paid to HTN, Lipids and the glycemic control which improves while there may not be substantial decrease in weight, and this achievement should be used as a marker for improved over all health rather than just on the weight numbers.
